In regards to the qualifications for QRP ... is it a combined income of $2000 US per household or per person?

Basically per household. One person in the household is the holder of the general QRP and the others are dependents. ie. Husband or wife signs up and the other spouse and children under 18 are dependents, so the $2000 US per month covers all of them.
That $2000US per month is what you have to put into the Belize bank every month (or alternatively $24000US per year). You need to be able to show that you have a regular, un-interrupted income (ie like a government pension) that will allow you to put this amount of money into the Belize bank.

1. How much does the application process cost?

2. How  long does it?

3. Can you start the process while on an extended visit?

4. What happens if you enter under QRP and your circumstances change and then you have to leave?

Here are the answers to your questions:

1. Fee payable at application submission:
USD $150.00 – Application Fee (non-refundable)

Fees payable at acceptance:
USD $1,000.00 – Applicant Program Fee
USD $750.00 each – Dependent Program Fee
USD $200.00 – QRP Program Card Fee

*Annual Renewal Fee BZD $50.00

2.This question isn't complete so I am assuming that you are wondering how long it takes to get it. I took about 3 months for them to process ours, but then it was about 6 months before we got our first card. However, as soon as you are approved you get a letter that can be used to identify that you are on the QRP program until you get your card.

3. Yes you can get it while on an extended visit. You do have to have all your paperwork from Canada when you apply, so if you plan to apply while on the visit, make sure you have everything with you. You still have to renew your passport visa every 30 days until you receive your confirmation of acceptance into the program. That is how we did it.

4. If your circumstances change and you have to leave, you just leave. If you want to maintain the program after you leave, you would need to renew your QRP every December and spend at least 1 month per year in Belize. If you don't want to keep it, you just don't renew it. However, if you have brought household and personal items into the country duty free under the program, you would have to pay the duty on them.
