Work visa in Baharain

I am a dentist( BDS) planning to travel on visit visa to Bahrain. I can apply for licence application only with employment offer letter. So will they ask me for CPR for the process of applying for licence exam as CPR card cannot be obtained with visit visa.
Secondly, once I clear the license exam, what is the next procedure to get work visa. Please guide me

I believe you need to have a job offer to apply for a licence with NHRA.  Once that is approved, only then you can ask your employer to apply for a work permit on your behalf.  Refer to this from NHRA:

Hi....I need to know what is the total expense an employer bears for issuing work visa for an employee for a period of 12 months...can anyone please help me to know this.

172 BD for 1 year, 344 BD for 2 years.  Including basic healthcare fees.  Plus monthly fee of 5 BD each month per permit up to 5 expats on CR and then 10 BD per month from 6th expat onwards.

What do you mean?

You only pay when the visa is approved not before.

Actually the thing is that my employer has already sent me a copy of visa but I donot want to join the company because I donot wish to relocate so that's why I am curious about their fees they have paid for my visa.

Ok.  They don't get any refund.   

Refund is only done when LMRA itself cancels the visa before arrival (100%) or the employer does mobility to another sponsor (varies depending on time period).
