You'll need the login info from the lawyer that did your initial Residency Visa application, and follow the instructions on   It gets confusing with which visa you actually are "renovating", so ask your lawyer that question, too.  But doing it online yourself isn't very difficult.  If you have a Dominican bank account, you can pay for most of the services online without having to run all over the place.  Prices change, but I think the Good Behavior was $600 RD, repatriation insurance was $120 US, pictures were $300 RD and the actual Residency card was $5,000 RD (I forget, but somewhere around that), and cedula was $4,000 RD.  After you upload everything, keep checking the site, as they don't send you an email when the status changes.  You don't have to accept the appointment the give you, it can be canceled and you need to input a reason.  The $1,000 RD fee is only if you accept an appointment and miss it, not to reschedule it online.
