How to get "Exit Re-entry Record" after leaving KSA

After working many years, I made my final exit from Saudi Arabia two years ago. My present country of residence asked me to submit "Exit Re-entry Record". I checked all websites but could not find any link. So, in this forum, anyone has this information that how to get exit re-entry record after leaving Saudi Arabia.
